Family & Children’s Agency Home Care’s Licensed Clinical Social Worker provides in-home therapeutic services for seniors and their family caregivers. Social work services also include referrals to community agencies, assistance completing necessary applications, as well as connecting seniors and their families to local resources and services.
Senior Outreach and Engagement
Our FCA social workers coordinate care for vulnerable and isolated adults age 55 and over with behavioral health issues including mental health challenges and substance abuse. This program is funded by the Department of Mental Health and Addiction Services (DMHAS).
Caregiver Support
Family & Children’s Agency Home Care’s Caregiver Support provides support to family members who are caring for aging adults.
Transportation Assistance
FCA provides drivers to transport seniors to and from doctors appointments and visits with friends and relatives. This program is funded partially by Southwestern Connecticut Agency on Aging (SWCAA).
